Referrals
Christa House accepts referrals by phone, fax, e-mail or
in person.
Referral Procedure
A potential resident or a representative is invited to call
the Director of Admissions at 631-376-0114 x 238.
The following information will be requested:
- The medical diagnosis and prognosis
of the referred person
- Name, address, phone
- Family and social support of the
referred person
- Financial support available to the
referred person
- An information cover sheet or a
Patient Review Instrument (PRI).
With appointment only, site visits are welcome from, Monday to Friday, 8:30am to
4:30pm. If the referred person is unable to visit Christa
House, the Christa House Admission Team will visit the referred
person.
At the time of this visit, information will be shared about
the Christa House Community and the services we provide. The
referred person will be assessed for suitability of placement
in Christa House.
Christa House will not accept anyone who does not wish
to be a resident of the house.
